  • Permanent role at a minimum of 0.8 FTE up to full time 1.0 FTE

About the Role and Service

  • NASC provides Needs Assessment & Service Coordination for older people and people with chronic health conditions with high needs who have a disability requiring long term supports, and for people of any age who require acute short-term community home support.
  • These positions, minimum of 0.8 FTE (32 hours per week) are based in either Whangārei, Bay of Islands or Dargaville Hospital with travel throughout Northland.
  • Primary functions of the role include providing screening and triaging of referrals and providing the coordination of the appropriate serviceds are identified in the need's assessment.

About you

Essential Skills & Experience

  • Tertiary qualification in either Nursing, Occupational Therapy, Social Work or Physiotherapy with a current practising certificate
  • Recent clinical experience with proven assessment skills
  • Excellent written and verbal communication
  • Advanced computer literacy
  • Clean and current NZ full driver licence
  • Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth
  • Experience in projects / initiatives which give effect to Te Tiriti principals and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings.

Desirable:

  • Working knowledge of services for older people and the disability sector
  • Working with Family/whanau in the community
  • Confidence of previously working autonomously and harmoniously within a multidisciplinary team
