Support our kiritaki/clients, their whānau and kaimahi hauora/support workers utilising your nursing expertise to develop individualised support services for those living with the life changing impact of complex injuries or disability and the training of those supporting them.

About the role:
We have an exciting opportunity for an experience registered nurse to join our passionate team based from our Palmerston North office. This role allows you to practice holistically with our kiritaki/clients, their whānau,and other health and disability services supporting them to achieve their goals and receive high quality and safe support services at home. Providing quality training and education of our kaimahi hauroa is a key component of this role. Applicants must be based in Manawatu/Horowhenua and have the right to work in NZ. 1FTE Monday - Friday.

Key responsibilities include:
* Developing and maintaining person-centred Individual Support Plans
* Managing homecare support programmes for kiritaki with complex life long injuries or disabilities.
* Ensuring our kaimahi hauora have the right skills, training and support to provide an exceptional level of support to our kiritaki.
* Facilitating, coordinating and delegating individual kaimahi hauora advanced clinical competency education.
* Facilitating efficient and effective community-based support solutions for our kiritaki
* Completing kiritaki related assessments
* Facilitating Kiritaki/whānau, MDT and support team hui as required
* Liaising with external support providers and funders including ACC, Mana Whaikaha, Te Whatu Ora, rehabilitation facilities and Allied Health services.
* Problem solving to develop innovative and sustainable support solutions
* Supporting quality improvement activities
Key competencies:
* A NZ registered nurse with a current practicing certificate and NZ context experience.
* 3+ year Experience working in community, complex disability and rehabilitation preferred.
* Current clinical competencies in management of PEG, stoma, SPC's, bowel and bladder support etc preferred but not essential.
* Experience providing training and education to others
* Full drivers' licence
* Able to use initiative, perform autonomously and as part of a team
* Ability to quickly develop rapport and trust with others
* Excellent communication and documentation skills
