About the Role

In this role you will work across all three of our teams – ICCT (Integrated Care and Co-ordination Team, ISR (Integrated Safety response Team) & Triage Services undertaking a huge programme of change to deliver better outcomes to tāngata whai ora and whānau in our region. About the Teams

The Integrated Care & Coordination Team (ICCT) is part of Adult Community Mental Health and Addiction Services (Te Kuuwaha o te Waiora). The ICCT team supports people with long term mental illness to transition from specialist mental health services to primary care, where their mental and physical health care needs can be met in an integrated way.

The Integrated Safety Response (ISR) health team is a Police-led agency that works collaboratively with key Government and non-Government agencies reviewing all incidents of family harm/family violence that is occurring and intervene with an integrated, cross sector safety plan to reduce the escalation of harm, prevent further harm and connect families and children with the support they need. This service is a 7 day a week service (0800-1630 hours).

The Triage service provides a single point of entry for non-urgent referrals to Adult Mental Health (Te Kuuwaha o te Waiora) between 8am to 4.30pm, 6 days a week. The service undertakes initial screening and assessment (telephone or face-to-face) of referrals to ascertain whether a comprehensive assessment is indicated within the community mental health service; or alternatively, to provide appropriate advice and/or facilitate an assisted referral to another service provider, such as a community based Non-Government Organisations (NGO’s).

About you

• Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
• Experience in upholding Te Tiriti principals and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings.
• Excellent communication and interpersonal skills.
• Dynamic and flexible with a can do attitude.
• Commitment to fostering partnerships and interagency collaboration.
• A passion for making a difference to the lives of our tangata whaiora and whanau.
• Be familiar with assessment of a wide range of mental health presentations / disorders.
• Demonstrated skills in effective documentation of clinical assessments and interventions, including formulation.
• The ability to work effectively in a team environment.
• Current full drivers licence.
• Must have registration, or be eligible for registration with the appropriate registration body.
