The Opportunity:
Get the best of both worlds – utilise your acute clinical skill sets in a rehabilitative community setting. With this role you will be part of developing and implementing an innovative new service model catered to provide a smoother transition for clients from hospital to home. As a core Occupational Therapist, you’ll be collaborating with other passionate clinicians to provide an interdisciplinary approach to patient care; enhancing health outcomes for clients and whaanau enabling them to reach their full potential. This team works across the five community bases delivering acute care to patients under hospital in the home, reablement and transitional care, with strong links with inpatient hospital services.
In this role you will:
• Apply knowledge, skills and clinical reasoning when providing community intervention
• Provide interdisciplinary practice based on a reablement philosophy
• Work with the allied health clinical leadership team to continuously evaluate the service to improve client outcomes
• Work with the wider Community Health Services and acute allied health teams to provide smooth transitions of care for patients

Essential:
• Registration and current APC with the NZ Occupational Therapy Board
• Minimum 2 years Occupational Therapist clinical experience
• Full, clean drivers’ licence
