Health Coach Community/Support Worker

About the Role
Right now, an exciting opportunity has arisen to join our new health initiative: the Integrated Primary Mental Health and Addiction Model. The role is at the heart of primary care within general practice and is committed to the health and wellbeing of individuals, groups, and whanau of all ages.
Location and Hours
This is a permanent, part-time role, covering the South Canterbury region (Twizel, Geraldine, Timaru, Temuka and Waimate). We are open to having more than one person fill this 0.7 FTE vacancy and we ask you to please detail your preferred working hours/FTE in your application.
About you
As an experienced support worker, the successful applicant will be working within General Practice as an integrated team member, assisting clients gain the knowledge, skills, and confidence to become active participants in their own care. All this is so that clients can reach their own self-identified health goals.
The successful candidate will have excellent verbal and written communication skills to carry out the intervention sessions and the associated reporting. Teamwork and relationship management are also critical to success.
If you are passionate about contributing to a multi-disciplinary approach to healthcare, this moment is yours. Applicants will have completed the level 4 certificate in mental health support work and addiction or a recognised equivalent qualification for this role.
A current full driver - s license is also essential for this vacancy given
the community facing nature of the role.
Specific Health Coach training will be given as part of this role.
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth
- Experience in projects / initiatives which give
effect to Te Tiriti principals and frameworks, and the application of
Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in
healthcare settings.

Salary Range
This role is covered by the terms and conditions of the Collective and the salary will be in accordance with the Pay Scale, and dependent on relevant experience.
